What is an authorized representative?
An authorized representative is a person that you may choose as someone to act on your behalf with the New York Independent Assessor Program.
An authorized representative can be a family member, a caregiver, or other person that you trust to make important decisions related to your health care. Some authorized representatives may have legal authority to act on your behalf.
To designate someone as your authorized representative, you must complete the Authorized Representative section of the New York Independent Assessor Program Information Sharing Consent Form, which must be completed and submitted online.
