In-person appointments

An in-person appointment means a nurse assessor or clinician will meet with the individual at home, a family member or friend's home or apartment, or at a facility where services are being received. The nurse assessor or clinician can see and talk to the individual in person.

Video Visits

A video visit is very similar to an in-person appointment, but instead of meeting face to face, the appointment will be conducted by video. The nurse assessor or clinician can see and talk to the individual using a smartphone, tablet, or laptop with a camera. For a video visit, the New York Independent Assessor Program uses an application called Zoom that is HIPAA compliant, which means it protects personal and health information. In order to receive a link to the video visit an individual must provide an email address or phone number for text notifications.
