Managed Long Term Care

Managed Long Term Care (MLTC) plans help provide community based long term services and supports (CBLTSS) to people with chronic health conditions or disabilities. These services, such as personal care services (PCS) or Consumer Directed Personal Assistance Services (CDPAS), are provided through MLTC plans. All MLTC plans are approved by the New York State Department of Health.

Individuals who are requesting enrollment into a Managed Long Term Care plan must get an assessment through the New York Independent Assessor Program. After the assessment is complete, a notice will be sent with the results.

Community Based Long Term Services and Supports

Community based long term services and supports are medical and non-medical services that are covered by Medicaid to help adults with disabilities or illnesses live safely in their homes and communities. These services and supports include:

  • Nursing services in the home 
  • Home health aide services  
  • Private duty nursing 
  • Personal care services in the home, such as bathing or dressing
  • Adult day health care 
  • Consumer directed personal assistance services  
  • Therapies in the home (physical, occupational, respiratory, or speech pathology)
