To receive personal care services and/or consumer directed personal assistance services or to request to enroll in a Managed Long Term Care plan, you must have Medicaid. Once you have Medicaid, you must have an assessment to see if you qualify for these services and if you can request to enroll in a Managed Long Term Care plan.

First: We schedule your community health assessment appointment and clinical appointment 

We will set up an in-person or video community health assessment appointment and a clinical appointment at a time that is convenient for you.

An in-person community health assessment appointment can take place at your home or another place where you will be receiving services, or at a hospital or another facility from which you will be discharged to return to the community.

We will also schedule a clinical appointment after your community health assessment appointment with a nurse assessor is completed. 

Second: Our nurse assessor conducts your community health assessment appointment  

Our nurse assessor will meet with you to discuss your health care needs. You will talk about activities you need help with every day. It will be important to have medical records from the doctors you are currently seeing. The nurse assessor will need this information.

Third: Our clinician conducts your clinical appointment

Medicaid requires a New York Independent Assessor Program clinician to conduct a medical exam. The clinician will discuss the services you need and whether you can coordinate your own care or have someone who can coordinate your care for you. During the clinical appointment, you will meet with a physician, nurse practitioner, or physician assistant.

Fourth: We tell you the results of your assessment 

You will receive a notice in the mail letting you know the results of the assessment. If you meet the requirements to request personal care services and/or consumer directed personal assistance services, your plan or Local Department of Social Services will arrange for these services. The notice will also tell you if you meet the requirements to request to enroll in a Managed Long Term Care plan
