The New York Independent Assessor Program is for New York Medicaid members seeking personal care services and/or consumer directed personal assistance services, or enrollment into a Managed Long Term Care plan.

How does the New York Independent Assessor Program help?

The New York Independent Assessor Program assesses New York Medicaid members to determine if they meet the requirements to request personal care services and/or consumer directed personal assistance services. The assessment helps us understand how a person's care needs affect their daily life. The assessment also helps determine if they meet the requirements to request to enroll into a Managed Long Term Care plan.

Who can the New York Independent Assessor Program help?

The New York Independent Assessor Program helps New Yorkers 18 years of age or older who have Medicaid, or both Medicaid and Medicare, find out if they are eligible to receive personal care services and/or consumer directed personal assistance services, or if they can request to enroll into a Managed Long Term Care plan.

Are there any costs for an assessment?

There is no cost to the individual to receive an assessment.
